Pfc Sam Kowalski attended high school for two years and was a stock clerk employed by the Phoenix Metal Ap. Co.

He enlisted in Chicago, Illinois on 22 September 1942.

In April 1944, he was hospitalized in Italy until June for artillery shell fragment wounds to his leg. From September 1944 until October, he was treated in France for a skin infection.

In January 1945, he was wounded in action by small arms fire to his thorax. He died of his wounds on 13 February 1945.
