Sgt Carlos M. Maldonado was a kitchen worker before he enlisted in New York City, New York on 19 February 1943.

Statement from 1st Lt Richard H. Gunn:
"Flying as deputy leader to Lt Simkins between 8500 and 9500ft, approximately eight minutes after leaving target, I noticed a burst of flak hit the lead ship (Lt Simkins). There was no noticeable immediate effect though the burst must have hit either the tail or waist gun position. Another burst hit the left engine, Lt Simkins started a slow climb out of position, continuing until a stall and then a spin. Control was gained then into a spin again. This happened twice. The airplane went into a final spin but I did see it go in."

Statement from Sgt William L. Peyton:
"I looked out of port waist window and saw five chutes already opened. At the same time I saw the ship in a slow downward spiral, and observed no smoke. I could not tell if props were spinning. I saw ship hit ground and burst into flames as though exploding."
