Sgt Samuel Wade volunteered for the Army of the United States on 28 October 1941 in Los Angeles, California.

The aircraft received two direct flak hits in the pilot's compartment and nose. It caught fire and went into a steep dive.

Five crew members were killed. They were initially buried at the cemetery of Lechenich.

The pilot was blown out of the plane and was the only survivor.
