Statement of Capt Enoch B. Stephenson who flew in the same mission: "Approximately 1315 hrs, Lt Carothers called and said that his engine has quit and that he was going to bail out. About five minutes later, he called gaspump for homing. I called him back and told him to steer 250 degrees. About two minutes later I called him on D channel and he said that he was going on channel B and call Mayday. That was the last thing that I heard of him."

The airplane probably crashed into the North Sea.
