Pvt Theodore E. Kulak graduated from Lincoln High School and was a sales clerk at a company that produced soda fountains.

He joined the Regular Army at Fort Hayes, Columbus Ohio on 21 October 1940.

He was killed by small arms fire in the thorax and abdomen.
